Consultation Fee
Mr Fazel’s fee for an initial consultation will not exceed £350.00 and his fee for any follow up consultations will not exceed £250.00. These estimates are correct as of the date of this letter but may increase. Please contact his office at admin@maisamfazel.com for up to date fees. This fee is payable irrespective of the duration of the consultation and applies to both face-to-face consultations as well as to telephone or video consultations. Please note we are unable to offer any medical advice via email. If you have further questions, please arrange a consultation via the office.
Please note some hospitals require you to provide a swipe of a credit or debit card on arrival. This will include insured patients. The hospitals may use this to recover their fees; it is not used to recover Mr Fazel’s consultation fees. Mr Fazel’s office may also separately ask for your card details in advance of your appointment to pay for any shortfalls not covered by your insurance policy.
Following your consultation, you may need certain tests (for example, blood tests or scans such as an X-ray, mammogram, ultrasound or an MRI) to help Mr Fazel diagnose your condition. If the test is undertaken by the clinic or hospital, and not by Mr Fazel, the fees for those tests will be determined by the clinic or hospital and charged to you, or your private medical insurer separately. You should ensure you have authorisation from your private medical insurer before proceeding with the tests. If you are self-funding the tests, it is your responsibility to establish the costs of these tests before proceeding.
If there are any additional professional fees which Mr Fazel will charge in relation to any of the tests he advises you to have, we will let you know what those will be.

Cancellation Fee
We require at least 48 hours’ written notice via email if you wish to reschedule or cancel your appointment without charge. The email must be sent to admin@maisamfazel.com and NOT to the hospital or clinic where the appointment is scheduled. For appointments on a Monday, we must receive notification before 11am on the preceding Thursday. For cancellations with less than 48 hours’ written notice or if you do not attend your appointment, a cancellation fee of £250 will be charged.
Please note that most insurers will not cover a cancellation fee, so you will be invoiced directly.
Private Medical Insurance
It is your responsibility to contact your insurance provider before your appointment to check the terms of your policy (particularly the level and type of outpatient cover you have, including any reimbursement limits on individual consultation fees). You will also need to provide us with your private medical insurance provider name and membership number, alongside your pre-authorisation number, ahead of your appointment. Mr Fazel is recognised by most of the leading Private Medical Insurance companies (including BUPA, AXA, Aviva, Vitality, WPA, CIGNA, CS Healthcare, Exeter, Benenden, Healthonline & Helix amongst others).
Please note you are responsible for any fees not covered by your insurer.
Chaperone
You may request a chaperone at any point during your consultation. If you have a remote appointment, you are welcome to arrange your own chaperone. Alternatively, we can arrange for a chaperone to be present virtually during your appointment. Please advise us in advance if this is required.
